In consideration of being
allowed to participate in any way in the Hickory Grove United Methodist Church
sports program, related events and activities, the undersigned acknowledges,
appreciates, and agrees that: (1) the risk of injury from the activities
involved in this program is significant, including the potential for permanent
paralysis and death, and while particular rules, equipment, and personal
discipline may reduce this risk, the risk of serious injury does exist; and (2)
I knowingly and freely assume all such risks, both known and unknown, even if
arising from the negligence of the releasees or
others, and assume full responsibility for my participation; and (3) I
willingly agree to comply with the stated and customary terms and conditions
for participation. If, however, I
observe any unusual significant hazard during my presence of participation, I
will remove myself from participation and bring such to the attention of the
nearest youth league official immediately; and (4) I, for myself and on
the behalf of my heirs, assigns, personal representatives and next of kin,
hereby release and hold harmless the Hickory Grove United Methodist Church,
their officers, officials, agents, and/or employees, other participants,
sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releasees") with respect to any and all injury,
disability, death, or loss or damage to person or property, whether arising
from the negligence of the releases or otherwise. I release and agree to
indemnify and hold harmless the Releasees from any
and all liabilities incident to my involvement or participation in these
programs as provided above, even if arising from my negligence. I promise to return all equipment issued to

Christian Coach’s Creed
A Christian coach
should exemplify Christ in their life and have a personal relationship with Him
on a daily basis. He/she should live a
life that is led and empowered by the Holy Spirit; therefore, they must be
consciously thinking God’s thoughts and consciously performing His way.
Their attitude must
reflect those of Jesus Christ and realize that they are an extension of their
church’s outreach ministry and that everyone around them, both members and
non-members alike, is constantly critiquing their actions.
A coach’s primary
goal must be to develop young people through athletics to become Christ-like, therefore, they must build self-esteem, motivate,
correct, and love each team member with the love of Christ.
Coach's Covenant
I am a Christian coach with a personal relationship with Jesus Christ.
As a coach I will share the gospel of Jesus Christ with the players and their families on my team.
As a coach I will always act in a Christ-like manner in dealing with players, opponents, parents, referees, and scorekeepers.
As a coach I will help each team member approach the game with the attitude of being a witness for Christ towards a teammate, opponent, and referee.
As a coach I will exercise self-control in displaying negative emotion or anger at a bad play, a bad call, or a questionable play by an opponent.
As a coach I will encourage and build up the players on my team at all times.
As a coach I will never berate, belittle, or embarrass my players during games or practices.
